CT NOFA Farm and Food Guide Submission Form

To be included in CT NOFA's Farm and Food Guide, please complete this form. If you represent more than one organization (for instance, if you run a farm but also are the manager for a farmers' market, or manage multiple farmers' markets), you'll have to independently fill out the form for each farm or market you wish to have listed. If you've previously filled out this form and would like to update your listing, you may submit the form again, in full, or email communications@ctnofa.org if you have a small change you would like to make. If you have any questions about or problems with the form, please email communications@ctnofa.org.

Transition to Organic Partnership Program (TOPP)(Required)
