To be included on CT NOFA's CSA Guide, please complete this form. If you've previously filled out this form and would like to update your listing, you may submit the form again, in full, or email communications@ctnofa.org if you have a small change you would like to make. If you have any questions about or problems with the form, please email communications@ctnofa.org.

CT NOFA Membership Status(Required)

Please choose the status that best describes your farm. Only choose "Transitioning to Organic" if you expect to become Certified Organic within the next 3 years. This information will not be displayed on the CSA Guide.
Transition to Organic Partnership Program (TOPP)(Required)
CT NOFA is a partner in the TOPP program, helping farmers in Connecticut transition their farms to Certified Organic status. Are you interested in learning more about this cost-free program? (If you're already a TOPP participant, please choose 'no.')
Farm Products Included in CSA(Required)
Please choose all products that your farm includes in any/all CSA options you have available.
CSA Pickup Locations(Required)
Please choose how many locations at which your CSA can be picked up during the season. We can only accommodate a total of 4 locations. If your CSA is available at more than this, please list your most prominent locations.
